Gina, I carry dermalogica products and they have a great consult form for the clients.  But I have also went on the ASCP website and used some of theres that are available for us.

We have 14 client forms for members to download on the ASCP website, plus forms in Spanish - once you sign up, you can find all of these on www.ascpskincare.com by logging into the member only area, and then you'll see a tab on the far right for Client Treatment Forms. Glad to have you as a member!

Thank you! Just spoke with one of the reps- so nice! (Nicole)- and got some questions answered. Looking forward to signing up in the next week (waiting to get a little closer to opening day so I have my full year in business).
