Client Feedback form
Does anyone have a Client feedback form? I am in desperate need of a in depth feedback form for my clients. On our ASCP site we have lots and lots of intake forms but no Feedback form.

Quick update on this: we're working on it now (two different versions, one for solo estheticians and one for a spa with multiple employees) and Kirsten should have them available on the ASCP website by the end of the week or early next week.
I think this is the second great idea for a new form that has been suggested by a member. The last one was the Lash & Brow Tinting consent form. So keep the ideas coming if there's another form you need!

Carrie can this form be edited by members because I am also an aromatherpist and I need to add specific questions for that. With my case studies I will need to ad question for that.

Good point - all of our forms are in PDF format. We will see about putting up Word document versions as well, so that members can use them as a starting point for their own customized versions.

Hi Teresa, we were talking about the Associated Skin Care Professionals website which is www.ascpskincare.com. The forms are an ASCP member benefit and are in the member-only area which requires a login.

The forms are only available to ASCP members and I'm afraid there are no plans to change that -- sorry!
