I've been an esthetician for eight months now, and I make it a practice to always have my client's file with me when entering the treatment room. I also makes notes for every client, whether they're new or repeat because I like to document my client's skin, the treatment I perform, any products I sell them, and sometimes personal info to help build the relationship. The spa I work in apparently doesn't think this is important, as I was once told that I'm the only esty that asks for one. I notice that most of the other estys don't use files or notes.
Our filing system is practically nonexistent, as you either search through overcrowded file cabinets, or a stack of files that is literally over three feet high. Since the front desk help usually can't find the client's file, they make them fill out the paperwork again. So, what is the general consensus out there on client files?
Also, should clients sign a short form upon a repeat visit stating that they are still in good mental and physical health and that their health/medications have not changed since their last visit?
Micro/chemical peel forms. Do you have your clients sign a pre-micro/peel form and a post peel form? One, both or no forms? We only have them sign a post-micro/chemical peel form which we keep.

Hi Lynn....it's really a must to have them sign a consent form. That consent form "has your back" if ever need be!

Depending on the state you are in it may be a requirement to have a clients signature and health history on file. Intake is really a necessity for a professional treatment and liability protection. I also use it as a sales tool, I take a photo of my client (with their permission) for a "before & after". I also will have a client sign an update on health history if I have not seen them for 6 months. I have a consent form (pre-service) for waxing, peels and microdermabrasion. ASCP has some great forms that you can download if you are a member. Sometimes your manufacturer has them as well and I am sure there will be other esty's here that have some that you can use. You are on the right track, a true professional!
