Hi Megan, there are two health history templates on the ASCP website for members - Confidential Client Health History for first-time clients, and Confidential Client Health History Update for a client's subsequent visits. Log in to the member area at www.ascpskincare.com and you will see a tab on the right called Client Treatment Forms. If these don't suit your needs, let us know, because we're adding new ones all the time.
