Intake forms

Hi, I was wondering if anyone has any ideas on re-accruing clients. My clients complain that they have to fill out all the paper work every time they come. Does anyone have updating forms? If so how does this work? I use The ASCP forms, however I haven't been able to personalize them. Has anyone had any luck with this also? I would like to put my logo on them and fill in the name section. TIA

copy and paste in a word program. you use what you need you could also save to your computer as a word program and edit it later.

I made my own forms. The front of the form has all the important information, on the back it asks if everything is still current and has a spot for them to sign and date and what service they're getting. It has that about 10 times on the back so they can just sign and date 10 times before filling out a whole sheet again if everything is still current.
