i have a client, first one to stump me.... its haunting my every thought and i need advice
she came in initally four months ago... very sensitive... using perscriotion retin a for 7 plus years daily, and clindamycin topical. her skin was thinned, and very sensitive... hence her products. she is also very fair, fitz 1. i told her to follow her dermatologists orders, but advised her that the topical meds are thinning her skin, and cause sensitivity. she ended up stopping both meds. she alao has hypothroid disease.
so, we have been trying some products... at first, i wanted to stimilate would healing, build her epidermis, and decrease sensitivity. after the first month she began breaking out badly. i assume it has alot to do with product change and she skin is resetting after stripping it for so long. her sensitivity has gotten better. before, the head wrap bruised her skin. that how thin it is. she does not get as red during facials. the texture has changed for better. but i know the breakout bother her.
hormonal acne is so difficult. i asked she shee her physician for a blood workup to get her throid levels. and see if her doc wants to add medication to balance them.
i had been seeinf 1 per month. for a mind enzyme, extractions, and glavanac current. now i have her coming in weekly for a galvanc treatment and extraction to get her cleaned out ans excellerat healing. i put her on a salicylic cleaning get, vit c serum, spot stop (helps to bring down inflammation and has sal). i also put her on a light moisture with healing peptides. all from circadia.
i know hormonal acne is tuff. she has to balance that with her doctor. she has minimal clogging if any, small pores, some dryness, not much. its definatly hormonal. she also does now follow home care. sleeps with makeip cause she is imbarassed for her boyfriend to see. the whole right side of her face erupted in cyctic papuals ans pustuals from sleeping with her head in bf s chest one night.
of corse im happy pass it to a drematologist, but what more can i do.
i nope to get an oxygen treatment in. i use a topical one, oxygen rx from circadia.... its amazing. our next plan is a raspberry enzyme from circadia as well.
im sure a led panel would help, but i dont have one....
any advise is welcome. i care so much about my ladies, and this has been my most challanging skin yet. i feel for her, cause i struggle with hashimotos, which is a hypothroid... and its horrible on the skin, and has taken me years to blalance it.

I would approach this situation in this manner:
1st. Do you have a relationship with a derm near you? If you do, is there a professional relationship where the derm will hand the client back for facial and skin care products? This is where the relationship part really comes in and its important to cultivate if you do not have. You will also need to be friends with derm's in house skin care professional. I think you see where this is going :)
I also believe that you would be able to ask the derm (privacy might be an issue here) or client for details and become smarter and more informed and on your way to the medical side of skin care.
2nd. The fact that you are willing to hand her to a derm is a really important point to commend you on. You care more for your client then money or ego. Kudos to you.
3rd. I always have said that as a skin care professional it is the clients we choose not to take that can and do at times make us the success we want to achieve. Only you can make the decision as to where this particular client falls within your practice and your current abilities.
I may have not directly helped you but I hope maybe given another angle to think about for you as a professional. Best of luck.

Does she see an endocrinologist? I think you are going to continue to struggle with her skin until she gets help with the underlying condition.
Nothing personal, but I think it was a mistake to discourage her from using her topicals... Ideally if you had to "rescue" her from the strong retinoid, it would have been better to switch in a lower-level retinoid to keep her from whiplashing into these heavy-duty breakouts that she is experiencing.
I think there's still time to get her topicals figured out. Remember, in addition to the actual products, it is so very important to make sure that she is using ice on any active blemishes. 1-2 minutes of ice per area, 2-3 times per day minimum (more if using ice helps make her comfortable).
What is your policy regarding seeing clients between facials? I think with this one that you may need to be open to seeing her for emergency extractions between facials. This will allow you to extract problem comedones before they blow up. She also needs to learn how to identify those comedones, use ice, and schedule tune-up appointments with you.
But at the end of the day, an endocrine specialist is going to be able to help control her thyroid levels and keep her safe and healthy (big cardiac risks for untreated hyperthyroid), and that treatment will have the side effect of making your job easier.
With all that said, if you are comfortable and within your scope of practice to offer her facials with extractions, I'm not convinced that involving a dermatologist will be necessary if she's open to working with an endocrinologist. The exception would be cystic lesions that need to be drained and/or injected with cortisone.
Finally, in addition to the exfoliants/retinoid that she needs to prevent getting clogged, I'd strongly recommend getting her into a product with Epidermal Growth Factor to help restore the barrier function of her skin.

i didnt discourage her off the topicals, quite tge oppsite. i was very clear that she follow her derms advice and treatment plan. i see her between facials for extraction and galvanac treatments... these are usually free if charge. i have ger on a growth factir as well. she has little to no clogging and tiny pores. her breakouts are mostly inflammed papuals. few rise to a white head. i also have her icing to decrease inflammation, to speed healing.

im pretty lucky. i got into aesthetics because i needed to find balance and peace in my working life. i find it here. i previoisly came from the mental health field. which was very difficult to leave at work. id never change the experience, but im too sensitive for that constant exoposure to abuse and pain. i worked with adolescents. im fortunate because my spouse is able to support us. my job now, is purely for peace in mind. im a people pleaser, and my love of aesthetics excites me beyound my expectations.
its about my clients. everything i make i put back into my practice. ive been successful, and just flumixed by this client i want so despertly to help. i relate to her. i will definatly look into a local derm to consult with. i have a few in mind. natually ill ask her

Even though you are questioning what to do, sounds like you've already gone far beyond what the majority of estheticians could or would do (sad to say).
I ha v e the same type of skin and what a t helped me was skin supplements. All s o don't over do it on products. That caused a lot of my break outs also. Try high frequency too. I love mine!
