Hi, How do you phrase your return policies?

I would like to structure a return policy that emphasizes the fact that each product has been selected by ME with knowledge of their skin, allergies, etc., and that returns are ONLY processed in the event of a DOCUMENTED skin reaction, meaning that they must notify me ASAP, take pictures, AND be willing to come in and allow me to examine the skin reaction WHILE IT IS STILL PRESENT.  

Can anyone give me examples of your wording?
