Does anyone have a good client treatment record form they use to keep up with all the services they perform on each client? I didn't see one in the list of forms provided by ASCP. I am currently using all their forms, if someone has one that fits with the other forms, that would be great.

ASCP has a form called the "progressive treatment record"  it is listed under client treatment records.

Hi John, I just wanted to let you know what I do.  I created a form online and I set up each of my client with a folder.  It has the medical history form, skin care form and progress notes for me to keep track of each treatment.  I set my file like a medical office as that is where I was introduced to skin care and it gives you detailed information on each of your clients.  It is easier to see progress or know if they have problem areas to focus on.  It also allows you to know if there are medical issues that you need to be aware of or allergies to products to keep track of.  I put allergies in red ink on the outside of file to remind me.  It will keep things professional and allow your client to feel confident that you know what your doing and keeping their information private.  Always follow privacy laws with your clients folders. 

With the abilities of computers and printers, you can customize your forms best suited for you!
