    Hi Luisa,

    If you're an ASCP member, you can log in to the Members section of www.ascpskincare.com, then click Business Tools on the purple toolbar at the top. You'll see Client Treatment Forms listed, along with all the other tools you can use as a member. We have a pretty large variety of forms, some very detailed for specific treatments. Let me know if that helps!
