Hi Gail, have you checked out the client forms in the member login area at www.ascpskincare.com? There are 14 forms you can use. We do not have a specific form for glycolic treatments, but the wording on some of the other forms might be enough of a starting point for you to customize your own. I'll look into whether this is a client form we should add in future.
