I am a new Licensed Esthetician and wanted to ask you experienced people, what do I need to include in the files that I make for my clients? I have an intake form with all the necessary questions that needs to be signed by the client, but was wondering how do you record your findings and conclusions after a face analysis? And what else is needed to be in a clients folder?

We are experimenting with a one-page form for each skin treatment. At the top we write in the date, service provider, and treatment to be performed, and the client signs consent to that day's treatment. Then, the rest of the form follows the steps of our treatments, and covers their skin condition, aggravating factors, lesion count (if treating acne), exfoliant used, minutes used, outcome of exfoliant, extraction notes, etc. throughout the whole treatment. We keep working on it to make it even more easy to use (i.e., just circle what you did), and then the receptionist puts the data from it into the system under client notes.
All below and any notes or follow up- if she/he tipped well ;) -kidding- what recommendations for future services...

In addition to all the important things like their medications, allergies, skin type, lifestyle, etc...I always note their skin condition, what my home care recommendation was and if I gave them any samples. Also to follow up on what treatment program I designed for them so I don't forget what I told them. Of course you've got to know what retail they bought to make sure they are using it and if they like it. Helps your retail tremendously when you can see they haven't bought a moisturizer in 6 months.
I keep the intake form and on the back I just write what I found after the analysis and how the treatment went if they bought any products, if the client shared anything about personal life and something to help me remember them and how the treatment went. The next time they come back to see me and I'm able to remember something like that that they talked about they think I have the best memory.
