opps, must've hit the wrong key.   . . . but we still have the minor monthly breakouts that are leaving hyperpigmentation.  I've counseled on diet, supplements, and other lifestyle issues but am super frustrated!!!

Joanna, I feel your pain. My biggest acne challenge is the monthly hormonal stuff that is non-inflamed and not extractable.  I have several I'm working with right now and another coming in for the first time Friday.  I've achieve major improvement with those I've been treating but

Thx, Mary Lou.  Its grade I, not severe.  Non-inflammatory.  Most of the lesions are small whiteish bumps that don't extract - there's no pus.  They will shrink when I use glycolic/lactic blend, followed by LED tx.  There are also some larger pink lesions that are not extractable.  

I have not advised any supplements, but I know she takes B12 and folic acid.  She does not eat seaweed, but does eat seafood, salt, etc.  She's more of an androgenic type.  I have her using a gentle gel cleanser, salicylic/enzyme blend twice per week (which she has not been good with) and a moisturizer designed to deal with hormonal breakouts.  I've also given her a serum for spot tx, but she says the lesions just come back after a few days. 

 

I appreciate everyone's help on this.

Yes, once addicted to Bikram, it's like giving up smoking.  What is the grade and severity of acne?  Inflammatory or not?  Do you have her on any supplements?  Is she a more androgenic or estrogenic body type?  Does she know to avoid iodides, salt, peanut products, beef liver, non-organic dairy, shellfish, wheat germ, etc.?  If she's Asian, kelp (seaweed) may be a part of her diet - bad news.

Thx, ladies.  They teach you in Bikram NOT to wipe the sweat, so I have her clean her face afterward.  She had this acne before the pregnancy.  She also has no intention of stopping Bikram.  Anyway, how can I clear this pregnant or not?  I'd still like to help her.

Well now that's she's pregnant she won't be doing bikram, so time will tell if it is aggravated by the heat.  I am also pregnant and recently went to an indoor water park, I think the combination of heat and chlorine makes my face look worse

How clean is the towel she uses to wipe the sweat off her face after yoga class?  Is it washed w/fabric softener?  And of course the 1st trimester of pregnancy often makes acne flare.  Heat and humidity in a Bikram class are so high.  There's a theory that the swelling and rigidity to the skin that occur in heat/humidity break down the follicle wall.

I currently have a client who has acne along her jaw.  She's Asian, age 42, eats well and exercises (including Bikram yoga several times per week).  The breakouts began after beginning Bikram, so I assumed it was hormonal acne aggravated by the heat of Bikram, and I asked her to clean the area after each workout with her home care cleanser, follow with her light moisturizer.  She sees me every 6 weeks, and we just finished a series of peels (gly/lac/enzyme blend), which would clear the acne for a few days after the peel, then come back slightly.  Now that the peel series is done, its more pervasive.  She is not terribly compliant about home care, and I know she'll mix products.  She absolutely does not believe that the Bikram has anything to do with this.  Any comments/ideas?  To complicate, she is currently pregnant, 1st trimester.
