The article doesn't address it, but I have heard that all consent forms need your business name, address, and phone # printed on them, along with signatures, to make them official documents if there is ever a legal question brought up against you. Does anyone know if this is true? I make sure my info is on everything, just in case, but I don't know if anyone else does this.
Hi Michelle, I usually try to capture all of the client's contact information just in case. I usually have my regular customers fill out an updated health history and consent after 6 mos. to a year as well.
