Independent Contractor Agreement

Hello everyone,

I am a Licensed Esthetician and own a small one treatment room skin care salon in San Diego.  I'm looking to bring in another Esthetician or Massage Therapist to work on my days off.  Does anyone have a "Independent Contractor Agreement" I could use as a template or know where I could find one?  Thank you so much!
